  1. The Scheme is promoted by Apollo Health & Lifestyle (AHLL) and is available to all persons who enrol in the Apollo Family Care program and are accepted for membership or to whom we make the scheme available.
  2. Membership of the program is entirely at our discretion, and accordingly, we reserve the right to refuse any applications.
  3. Complete name (first name and last name) of the individual along with his mobile number and Unique health ID (UHID) are mandatory for enrolling in the program.
  4. Members can earn or redeem their reward points (called "Apollo Hearts") at any participating Apollo clinics across India.
  5. No reward points will be allocated for services availed outside Apollo clinics or in other Apollo Healthcare service unit i.e. Apollo Cradle, Apollo Sugar Clinics, Apollo Day Surgery.
  6. The validity of reward points accumulated shall be two year (a 'cycle') from the date of earning them.
  7. The reward points accumulated cannot be carried forward to the next cycle. Periodic updates on the number of 'Reward Points accumulated' and 'Reward Points nearing expiry' shall be sent to members on their mobile numbers through text messages.
  8. The membership of Apollo Family Care program along with its benefits & privileges are non-transferable and cannot be sold.
  9. The member shall present his registered mobile number or UHID for all transactions & activities related to Reward pointsaccumulation, redemptions & discounts.
  10. All reward points accumulated will be credited to the member's account at the time of mid-night of the date of billing unless the system is down, in which case updating will be done when the system is available in working condition.
  11. AHLL shall not be liable for reward pointswhich were accumulated inaccurately as a result of technical malfunction, operator fault, member misconduct or any reason beyond the control of AHLL.
  12. Members can check their balance redeemable reward points at any Apollo clinics cash counter or/and in our program website or/and the mobile application.
  13. The reward points cannot be redeemed or exchanged for cash. No cash claim can be made in lieu of reward points/ gift vouchers/reward coupons.
  14. Reward Points of different members or other loyalty programs cannot be clubbed for redemption.
  15. AHLL assures complete privacy of your personal data and your mobile number. It shall not be shared with anyone without prior permission.
  16. AHLL reserves the right to amend, alter, modify, add or delete any benefits and privileges, terms and conditions of this scheme and Apollo Family Care program benefits from time to time without prior notice.
  17. Membership can/shall be terminated under the following situations -
    • Breach of any membership terms and conditions.
    • Behaves in an unruly manner while in Apollo clinics premises.
    • Provides false information while enrolling for membership.
    • Purchases in the name of firms or any third parties.
  18. Membership constitutes permission forAHLL to use the customer's name and comments for promotional purposes in any media without any compensation whatsoever.
  19. AHLL does not make any representation or warranty as to quality or suitability of the gifts/ rewards in any manner.
  20. AHLL shall not be liable for any loss or damage whatsoever that may be suffered or any personal injury that may be caused as a result of participation in the program or using the Reward Points / gift vouchers/ reward coupons.
  21. All disputes are subject to the jurisdiction of the competent courts at Hyderabad.
